To have training certificates re-issued:
Proactive Shooters will re-issue a training certificate for a student that has lost their original. There is a $5.00 charge for re-issued certificates. In order to request a certificate;
1) First send us a note telling us that you need a certificate re-issued, using the contact form below. Please indicate your name, which class is was that you completed, and the class date if possible (as close as you can recall). Please also indicate your current, correct mailing address,
2) You may complete the $5.00 payment by either mailing a check or money order to our mailing address, or by utilizing Paypal.com (our credit card processor). If using Paypal, you may send a one time credit card payment of $5.00 to training@ProactiveShooters.com.
3) Once we receive your payment and verify your attendance in class, we will re-issue the certificate and mail it out to you as soon as possible.
